What should I wear to my appointment?

Opt for comfortable clothing that allows easy access to your upper arm and feet/ankles, as we will check blood pressure, circulation, and nerve function. Avoid full-length nylon stockings if possible.

What should I bring with me?

Please bring a list of your current medication, the medication themselves, a notebook and pen for any notes, and any questions or concerns you wish to discuss. Additionally, bring any recent lab results, scan reports, or ER information.Opt for comfortable clothing that allows easy access to your upper arm and feet/ankles, as we will check blood pressure, circulation, and nerve function. Who will I meet during my visit?

A licensed Nurse Practitioner and possibly a Physician Assistant.

What will my appointment include?

You will discuss your current medications, allergies, family history, personal medical history, and any recent ER visits or hospitalizations. We will review safety, including fall risks and medical equipment needs, and conduct a variety of health checks such as blood pressure, heart rate, oxygen saturation, height, weight, and more. Depending on your situation, further screenings may include circulatory issues, nervous system function, bone density, and diabetes review.

How long will this appointment take?

We dedicate one hour per patient to ensure we have ample time to address all your needs, answer your questions, and cover important details thoroughly.

What happens after my appointment?

An electronic summary of your visit will be sent to your Primary Care Physician and insurance company. You wonโ€™t need to take any further action unless instructed otherwise. Note that while our Nurse Practitioners can prescribe medication, your Primary Care Physician will handle prescriptions as they are more familiar with your medical history.

You are welcome to bring a trusted companion if it helps you feel more comfortable or if you need assistance with translations or note-taking.
